Where is my data? Only on your iPhone, in an encrypted database. There is no account and no server.
How do I move to a new iPhone? Settings > Backup > Save a backup file, and write down the recovery key shown there. On the new phone, restore from that file with the key.
I lost my recovery key. Without the key a backup file cannot be opened. Nobody, including the developer, can recover it.
How do I delete everything? Delete the app. Data saved to Apple Health is removed in the Health app.
